Now Hiring: Per Diem Registered Nurse (RN) – Home Health Care Trinity Home Health Care Are you a compassionate and dependable Registered Nurse looking for a flexible schedule and the opportunity to make a meaningful difference in patients’ lives? Trinity Home Health Care is seeking an experienced Per Diem RN to join our growing team. Responsibilities:

• Complete Start of Care (SOC) and admission assessments
• Perform routine supervisory visits for home health aides
• Provide skilled nursing visits in patients’ homes
• Develop, implement, and update individualized care plans
• Collaborate with physicians, caregivers, and interdisciplinary team members
• Complete accurate and timely electronic documentation
• Educate patients and families on disease management, medications, and home care

Qualifications:

• Current Registered Nurse (RN) license
• Minimum of one year of nursing experience (home health experience preferred)
• Excellent assessment and communication skills
• Reliable transportation and valid driver’s license
• Current CPR certification
• Ability to work independently and manage a flexible schedule
